Referral Procedure
If a referral is required by your insurance company, make sure you call our office with all the required information. This includes:  specialist or service name, phone number and appointment date. Please allow three (3) business days for referrals to be completed.
Payment

We ask that you provide your current insurance information at every visit, making sure we have your most updated insurance information. Copays will be collected at the time of service. We accept MasterCard, Visa and Discover for your convenience. You may contact our billing office with any questions.  A copy of our financial policy is available upon request.
